How to submit an IT ticket

For all CPC staff. Use one short form for any IT problem or request. The form sends your request to the right IT person. It takes about 2 minutes.

Words in a box, like Submit, are what you see on the screen.

Step 1: Open the form

  1. Tap or click the button. The form opens in a new tab.

  2. Check that the title says IT Ticket Submission Form.

On the form, you must answer every question with a red *. This guide marks those questions Required. You can skip questions marked Optional.

Step 2: Say who and where

  1. NameRequired

    Type your first and last name.

  2. EmailRequired

    Type your work email.

  3. Phone Number/ExtensionOptional

    Add your phone number or extension so IT can call you back.

  4. CPC DivisionRequired

    Tap or click the list. Pick your division:

    • Brooklyn Community Services
    • CDS Sites
    • CPC Administration
    • Manhattan Community Services
    • Non Center Based ECS Sites
    • Queens Community Services
    • Senior Services
    • Other

  1. Your division's listRequired

    A second list appears. It has the same name as the division you picked. Pick your site, program, or department in it.

Example: picking Queens Community Services adds a Queens Community Services dropdown.

Tip: Can't find your site or department?

Pick Other. A box called Department, If not listed above appears. It is Required. Type your site or department there.

Important: Pick the right division and site.

The ticket goes to an IT person based on these two answers. A wrong answer sends it to the wrong person.

Step 3: Describe the issue

  1. Asset Tag NumberOptional

    Is the problem with a device? Type its asset tag number. It is on the bottom of a laptop, or somewhere on the device.

  2. Description of IssueRequired

    Say what is wrong. You can type up to 2,000 characters.

  3. FileOptional

    To add a photo or screenshot, tap or click Choose a file to upload. Pictures only.

  4. If applicable, has the problem happened to anyone elseOptional

    Pick Yes, No, or N/A.

A description IT can act on

  • What you were trying to do
  • What happened instead, with the exact error message
  • When it started
  • Where you are: room or floor

Step 4: Submit

  1. Scroll to the bottom of the form.

  2. Tap or click Submit.

Tip: If the form will not submit

A required question is still empty. Scroll up. Check every question with a red *.

What happens next

  1. You tap or click Submit.
  2. A ticket is created on the IT board.
  3. It's assigned by division and site.
  4. That IT person gets a notification.
